What is the Independent Living programme?

This is a weekly group education programme for people with chronic conditions who are over the age of 65 years. The aim of the programme is to promote health and well-being at home and in the community.

What are the contents of the programme?

The group education consists of a range of presentations on different topics. These topics were selected on the basis of their impact on health and well-being. The sessions are:
• Aging and activity participation
• Exercise and aging
• Healthy eating
• Safety at home (crime, fire safety)
• Falls prevention
• Medications management
• Community services and entitlements
• Stress Management
• Housing Grants

Why was this programme set up?

This programme was set-up because as people get older performing activities that are essential for their well-being become increasingly difficult.

People who take part in different activities and live healthy lifestyles manage their chronic conditions well and are less likely to be hospitalised.

People who took part in this programme have found it beneficial; their confidence and motivation improved, promoting engagement with meaningful activities.

Who will benefit from this programme?

To take part in the programme you must be over the age of 65 years and living at home.

Who runs the programme?

The programme was developed and facilitated by the Occupational Therapy Service in Louth Primary Care Services. We will have presentation from members of the Primary Care Multidisciplinary Team and other services such as Pharmacist, Dietician, Crime Prevention Officer, Fire Safety Officer, Age Action Ireland and Louth County council.
